- Choose Upheal if most of your clients are private pay and AI notes matter most to you. Its full EHR costs $1 a session or $99 a month flat and AI notes are included (prices as of October 27, 2026).
- Choose TherapyNotes if your practice relies on insurance billing, prescribing or MIPS reporting, or needs HITRUST. Solo costs $69 a month. TherapyFuel AI notes add $40 a month per clinician.
- TherapyNotes Group rises to $89 for the first clinician plus $60 per additional clinician on December 1, 2026. Upheal sets group prices by quote.
On the Solo plan, the TherapyNotes EHR (electronic health record) costs $69 a month, and TherapyFuel AI notes cost $40 more. Upheal writes notes from your sessions with AI and includes them in its own EHR, which costs $1 a session or $99 a month flat. That makes Upheal cheaper at every caseload for a solo clinician who wants AI notes. Upheal’s insurance billing is labeled new, and TherapyNotes has sold its EHR with insurance claims built in since 2011.

Choose Upheal if
Upheal’s pricing and AI notes favor clinicians paid mostly by their own clients. Our Upheal review covers the full feature set.
- Your clients mostly pay out of pocket. At 80 sessions a month, the Upheal EHR costs $80. TherapyNotes Solo with TherapyFuel costs $109.
- Your caseload changes month to month. Pay-as-you-go has no minimum charge. No-shows removed from the calendar aren’t billed.
- You see couples or families. Upheal’s paid plans write AI notes for both. TherapyFuel covers individual sessions only.
- You meet clients on Zoom, Google Meet or another video tool. Upheal can record from any browser tab. TherapyFuel Scribe works only inside TherapyNotes.
- You use TherapyNotes and only want better AI notes. Upheal’s $39 AI Notes plan runs beside TherapyNotes. Its Smart Fill extension copies the notes into TherapyNotes.
Choose TherapyNotes if
TherapyNotes is the stronger pick for practices that bill insurance or prescribe. Upheal’s own comparison page recommends TherapyNotes for insurance-heavy practices, prescribers, MIPS reporting and HITRUST.7
- Insurance pays for most of your sessions. Claims reach over 2,800 payers. Secondary claims go out electronically.
- You prescribe. ePrescribe costs $65 a month per prescriber and covers controlled substances. Upheal has no prescribing feature.
- You report to MIPS. Reporting costs $359 a year per clinician. Upheal offers none.
- A contract requires HITRUST. TherapyNotes holds HITRUST certification. Upheal does not.
- You run therapy groups. The Group DAP note wizard carries shared details into each attendee’s note.
- You want phone support at any hour. TherapyNotes answers the phone 24/7, free.
- You employ billers or other office staff. Non-clinical users are free on Group.

Monthly cost at 40, 80 and 120 sessions
Upheal’s EHR cost rises with your caseload. TherapyNotes Solo costs the same every month. These totals include AI notes and leave out claim and card fees (prices as of October 27, 2026).
- Upheal EHR: 40 × $1 = $40. 80 × $1 = $80. At 120 sessions, EHR Pro at $99 flat beats $120 on pay-as-you-go.
- TherapyNotes Solo with TherapyFuel: $69 + $40 = $109 at every caseload.
EHR Pro also includes Assistant Pro, which otherwise costs $39 as an add-on. Our Upheal pricing guide covers every plan.
A two-clinician TherapyNotes practice with TherapyFuel pays $79 + $50 + $80 = $209 a month until December 1, 2026. From December 1, the bill is $89 + $60 + $80 = $229.1 Upheal charges $1 a session per provider. It quotes group prices on request.
Both platforms charge claim fees on top of the subscription. At 80 insured sessions, TherapyNotes charges $22.40 today, at 14 cents per claim plus 14 cents for each claim in an ERA. From December 1, the same month costs $27.20. Upheal charges $24, at 30 cents a claim. Upheal’s comparison page says the claim price includes the ERA.7
Insurance billing
TherapyNotes has the longer-established claims system. Its clearinghouse, Claim.MD, reaches over 2,800 payers. Secondary claims go out electronically once the primary ERA is posted.
Upheal’s insurance billing comes with both EHR plans and is labeled new. Its built-in clearinghouse covers commercial plans, Medicaid and Medicare. You bill under your own NPI and can bill primary, secondary and tertiary insurance.
Neither subscription includes credentialing. TherapyNotes refers you to a credentialing partner at varying prices. Upheal offers it through Managed Billing, a done-for-you service that costs 5.9% of what Upheal collects. Upheal does not publish the credentialing fee. It credits the fee back against billing fees over your first six months, for up to five applications.
AI notes compared
TherapyFuel works only inside TherapyNotes. Upheal captures sessions from most video tools and in person. It also writes notes for couples and families.
How each captures a session
Upheal records in-person sessions on phones, tablets and Macs through its Recorder app, or in a browser. Online, it captures any browser tab or desktop app, connects through its Zoom app, or records in its own telehealth. Dictation and uploaded recordings also work. Upheal has ten built-in note formats, among them SOAP, DAP and BIRP, plus 170 sections for building your own template.
TherapyFuel Scribe records sessions held in person or in TherapyNotes Telehealth. Scribe can’t record on outside video platforms or accept uploaded transcripts. A recording must run at least 8 minutes. TherapyFuel can also draft a note from your written summary.
Consent, AI training and retention
Upheal requires client consent before any client data enters the platform. Consent is collected by portal form or recorded offline. TherapyNotes supplies a customizable AI consent form. The company says it can’t guarantee the template fits every practice.
Upheal may use de-identified data from a clinician’s sessions to improve its AI until the clinician turns that setting off. The client must have agreed as well. Turning the setting off takes a paid plan, though clients can opt out on any plan.8 TherapyNotes says TherapyFuel runs on a vendor’s standard AI model. That model does not learn from how clinicians use TherapyFuel.9
Upheal deletes recordings by default but keeps transcripts unless you turn on automatic deletion. TherapyFuel deletes the audio once transcription succeeds and keeps transcripts until the note is signed or 30 days pass.
Accuracy record
Upheal’s help center warns that its AI may invent content for a note section the session didn’t cover. The help center calls each note a starting point. Alma built its Note Assist tool with Upheal. Note Assist had an issue in early December 2024. About 1% of the notes it had written since its launch contained content never discussed in session.10 The companies said they fixed it in under two business days. TherapyNotes tells users to review all TherapyFuel output. We found no public TherapyFuel accuracy incident as of October 2026.
Using Upheal with TherapyNotes, or switching
Upheal’s AI Notes plan can run beside TherapyNotes. Smart Fill, a Chrome extension, copies an Upheal note into the TherapyNotes form when section titles match. TherapyNotes Solo at $69 plus Upheal AI Notes at $39 comes to $108 a month. Solo with TherapyFuel costs $109.
TherapyNotes has no bulk export for clinical records. Upheal’s guide has you export the client list as one spreadsheet, then each chart as a PDF, one client at a time. Unsigned notes may be left out. Upheal’s import is self-serve or done with a migration specialist.
What users say about Upheal and TherapyNotes
TherapyNotes holds 4.9 on Trustpilot from 3,224 reviews. Of those, 574 came in the past year.5 Its newest reviews mostly praise support. Capterra reviewers rate it 4.7 from 972 reviews and call it easy to learn.6 Recurring complaints are add-on costs, yearly price rises and problems with the client portal. TherapyNotes Mobile scores 2.73 from 40 App Store ratings and 2.0 from 19 on Google Play.
Upheal’s Trustpilot score is 4.6 from 103 reviews, and the company has replied to every negative review.4 Reviewers most often praise the quality of the notes, the time they save and how fast support answers. Complaints cover note wording that needs fixing and Assistant errors. In therapist forums, clinicians who tried several AI scribes have preferred Upheal for notes tied to treatment-plan goals. A clinician reviewer quoted in the 2025 ClearHealthCosts report named Upheal the AI note tool she would be least likely to recommend. She cited wordy notes, omissions and errors.10
When neither one fits
Neither platform puts a long-tested claims system and AI notes for couples and families in one product. An insurance-heavy practice that wants both can bill through TherapyNotes and add Upheal’s AI Notes plan beside it. A practice choosing a different EHR for billing could look at SimplePractice, which Upheal’s own comparison credits with years of running insurance workflows. Upheal’s Smart Fill also copies notes into SimplePractice and Jane. Our best mental health EHR guide compares more platforms.
Start with Upheal’s free trial if most of your clients pay privately and you want AI notes from most video tools. TherapyNotes is the better fit if your practice relies on insurance billing or prescribing, or needs HITRUST.
