At a glance
  • Tirzepatide requires a prescription. A telehealth clinician can prescribe it because it is not a controlled substance.1
  • LillyDirect, Eli Lilly’s online pharmacy, offers self-pay brand Zepbound vials starting at $299 a month. It fills prescriptions but does not write them.
  • Compounded tirzepatide is not FDA approved. Since March 2025, pharmacies may make it only for individual patients in limited cases.2

Getting tirzepatide online involves two steps: finding a clinician who can evaluate you and choosing a pharmacy to fill the prescription. A video or chat visit may lead to a prescription, which you can have delivered or pick up at a pharmacy.

Eli Lilly makes tirzepatide as a once-weekly injection under two brand names. Zepbound is used for weight management and sleep apnea, while Mounjaro is used for type 2 diabetes.

Sesame Care offers video visits with providers who prescribe brand Zepbound and send self-pay prescriptions to LillyDirect. Hims, Ro and WeightWatchers also offer only brand-name drugs. Other services sell compounded versions. Below, we explain the options and what you pay for the visit and medication.

Which tirzepatide products can you order online?

Zepbound and Mounjaro are the only FDA-approved tirzepatide products. All approved forms are injected under the skin. There is no FDA-approved tirzepatide pill.3

Zepbound is approved for adults with obesity or overweight and at least one weight-related condition. Its approval includes use alongside a reduced-calorie diet and increased activity. It is also approved for moderate to severe obstructive sleep apnea in adults with obesity.1

Zepbound comes in single-dose vials, a multi-dose KwikPen and single-dose pens.

Mounjaro is approved to improve blood sugar control in people ages 10 and older with type 2 diabetes.4 It is not approved for weight loss. A clinician can prescribe it for that purpose off-label, but the decision is theirs.

Our guide on how to get Mounjaro for weight loss explains that option. Our tirzepatide guide covers how the medication works in the body.

How to get tirzepatide online, step by step

Most programs start with a health questionnaire and clinician visit. If needed, you complete lab work before a prescription is sent to a pharmacy. Ro says it can determine eligibility within 2 days.

Intake and eligibility

You answer questions about your weight history, health and goals. A clinician then reviews that information with you by video or chat.

The FDA’s 2023 Zepbound approval covered adults with a BMI of 30 or higher, or a BMI of at least 27 with a condition such as high blood pressure or type 2 diabetes.5 The label excludes people with a personal or family history of medullary thyroid carcinoma or MEN 2.1

The prescriber decides whether treatment is appropriate. No service guarantees a prescription.

Lab work before a prescription

Your clinician may order blood work before prescribing. Sesame Care includes Quest labs when metabolic data is needed, except in Arizona, Hawaii, North Dakota, New Jersey, New York, Oklahoma, Rhode Island, South Dakota and Wyoming.

Your prescriber chooses the dose, which appears on the pharmacy label.

Pharmacy and shipping

Self-pay Zepbound vials are available only through LillyDirect, with free delivery or free pickup at Walmart Pharmacy. Insured Zepbound pens can be filled through a retail or mail-order pharmacy.

Compounded tirzepatide comes from the compounding pharmacy used by the telehealth service. Henry Meds says a first shipment usually takes 8 to 10 business days after the provider visit.

The FDA advises against using an injectable GLP-1 that arrives warm or without enough cooling.6

Ordering brand Zepbound and Mounjaro from LillyDirect

LillyDirect fills prescriptions for Lilly medications but does not prescribe them. Your doctor or telehealth clinician sends the prescription to LillyDirect Pharmacy. A pharmacy partner then contacts you to confirm your information, check savings and arrange delivery or pickup.

LillyDirect offers four forms:

  • Zepbound vials: Self-pay only. A month’s supply contains four weekly vials. Needles and syringes cost extra. The lower prices for doses from 7.5 mg to 15 mg require a refill within 45 days.
  • Zepbound KwikPen: One multi-dose pen contains four weekly doses. You can have it delivered or pick it up at Walmart Pharmacy.
  • Zepbound single-dose pen: Available through insurance, with a savings card for eligible commercial plans.
  • Mounjaro pen: Available for self-pay at the same price for every dose. Lilly’s page lists it for type 2 diabetes.

LillyDirect’s homepage links to independent prescribers, including General Medicine, Counsel Health, Sesame, WeightWatchers and knownwell.

Best online tirzepatide providers compared

Five of the eight options below offer only brand-name drugs. Three offer compounded versions. Nearly all charge separately for the provider and medication.

Provider Tirzepatide offered Provider fee Medication
Sesame Care Brand Zepbound and Mounjaro $59 a month (annual plan) or $99 month to month Separate; self-pay Zepbound via LillyDirect
Hims Brand Zepbound vial and KwikPen, Mounjaro $39 first month, then $149 a month Zepbound vial or KwikPen from $299 a month
Ro Brand only, including Zepbound $39 first month, then $74 a month (annual plan) Separate
WeightWatchers (WW Med+) Brand Zepbound, no compounds $25 first month, then $74 a month (12-month plan) Zepbound from $299 a month
Noom Med Brand Zepbound vials; compounded microdose tirzepatide Brand: $39, then $129 a month Zepbound vials from $299 a month; compounded plans from $79 with medication
Mochi Health Compounded injection, brand Zepbound and Mounjaro $199 for 12 months (insurance plan) Compounded from $90 a weekly dose
Henry Meds Compounded oral tablets only Included $1,047 per plan, $891 paid in full
LillyDirect (pharmacy only) Brand Zepbound and Mounjaro None; bring your own prescriber Zepbound vials $299 to $449 (refilled within 45 days); Mounjaro $499; insured Zepbound pen as low as $25

Prices are as of October 19, 2026. Self-pay Zepbound costs the same through every service that sends prescriptions to LillyDirect. The provider fee is the main price difference between brand-name programs.

Among the four brand-only memberships, Sesame Care’s annual plan has the lowest monthly fee. It includes video visits, unlimited messaging and labs in most states. Some Sesame providers charge more than the base rate.

Sesame visits are self-pay, but providers complete prior authorization paperwork for covered medications. Our Sesame Care review explains the platform in more detail.

Choose a Sesame Care provider for a brand Zepbound visit

In Trustpilot and BBB reviews read during September and October 2026, the most common complaint about weight-loss programs was a membership fee added to the advertised medication price. Some reviewers said they paid before learning at the visit that they did not qualify.

Sesame offers a full refund only if you cancel at least 3 hours before the first visit.

Tirzepatide online with no subscription

You can get an online tirzepatide prescription without a monthly membership. General Medicine and Counsel Health are among LillyDirect’s prescribers that charge per visit.

Optum Perks accepts a one-time prescription request without an appointment or monthly fee. Its options include the Zepbound KwikPen. Medication costs extra with all three services.

Where to get compounded tirzepatide online

Compounded tirzepatide is not FDA approved. The FDA says compounded drugs are intended for patients whose needs cannot be met by an FDA-approved medication.6

The FDA declared the tirzepatide shortage over in December 2024. Grace periods for copies made during the shortage ended in March 2025.

A state-licensed 503A pharmacy can still prepare it for an individual patient if the prescriber documents a change that makes a significant difference for that person. For now, the FDA also will not take action against a compounder filling no more than four prescriptions for a copy each month.2

Three services in the comparison offer compounded tirzepatide:

  • Mochi Health: A once-weekly compounded injection.
  • Noom Med: Compounded microdose plans with medication included.
  • Henry Meds: A once-daily compounded tablet only. No oral tirzepatide has FDA approval.3

Sesame Care, Hims, Ro and WeightWatchers do not sell compounded tirzepatide.

How to check a tirzepatide compounding pharmacy online

Henry Meds and Mochi Health did not name their compounding pharmacies on the pages we reviewed. Ask which pharmacy will fill your prescription, then check its credentials.

The FDA’s BeSafeRx page links to every state board of pharmacy’s license database. The agency advises against using an online pharmacy that is not listed.7

The National Association of Boards of Pharmacy (NABP) says a legitimate pharmacy must be licensed both where it operates and where the patient lives. Check your own state’s database, too.8

According to the FDA, a safe online pharmacy also:

  • Requires a prescription.
  • Provides a U.S. address and phone number.
  • Has a licensed pharmacist available to answer questions.

The FDA publishes a list of registered 503B outsourcing facilities and their inspection dates.9 These facilities cannot currently compound tirzepatide from bulk ingredients.2

PCAB accreditation from ACHC is a voluntary quality credential, separate from a pharmacy license. We did not find a public lookup, so ask the pharmacy for its certificate.

Warning signs of illegal sellers and research peptide sites

According to NABP, nearly 95% of websites offering prescription-only medication operate illegally. Among the illegal pharmacies it reviewed, 96% did not require a valid prescription.8

The FDA has also found fake compounded tirzepatide labeled with the names of pharmacies that do not exist.6

Its warning signs for online GLP-1 sellers include:6

  • Claims that a compounded product is equivalent to an approved brand such as Zepbound.
  • Deep discounts that seem unrealistic.
  • No screening or prescription from a licensed clinician.
  • Products that look unfamiliar, arrive broken or lack instructions.
  • Labels with misspellings, an incorrect pharmacy address or a pharmacy name that appears fake.

The FDA has warned research peptide websites that sell tirzepatide labeled “for research purposes” or “not for human consumption” while providing dosing instructions. It urges consumers not to buy these products because their quality is unknown.6

NABP also warns against websites that accept only cryptocurrency or peer-to-peer payments. Its Safe Site Search tool lets you check a web address. Websites ending in .pharmacy are NABP accredited.8

Product problems can be reported through the FDA’s MedWatch program.

Self-pay Zepbound prescriptions from Sesame Care are filled through LillyDirect, Lilly’s pharmacy. Vials include free delivery or Walmart pickup.

Start a Sesame Care weight-loss visit for brand Zepbound

Tahara DeBarrows, LMFT headshot Tahara DeBarrows, LMFT

As a therapist, I would encourage someone considering an online weight-loss program to notice how comfortable they feel asking questions. I would want a provider who makes room to discuss concerns and explains what happens if treatment is not appropriate. Paying for a visit should not make someone feel obligated to move forward.

I would also encourage separating personal goals from pressure about how quickly weight should change. My focus would be helping someone clarify what they want from treatment and what support they need along the way. Decisions about tirzepatide itself belong with the prescribing clinician.
