At a glance
  • Solo plans run $49 (Starter), $79 (Essential), or $99 (Plus) a month, billed on a month-to-month basis. Groups pay $99 for the first clinician and $74 per additional clinician on Plus.
  • Additional fees include insurance claims, processing fees, and American Medical Association (AMA) CPT charge. Insurance claims beyond the included number in the selected plan are $0.50 per claim. Card processing fees run 3.15% + $0.30. Every clinician pays a $20 annual AMA CPT fee for billing and charting access.
  • SP does offer a 30-day free trial with no requirement for a card. Users are rewarded for starting a paid plan within the trial’s first 7 days by reducing the billing fee to 50% off for 3 months.

SimplePractice (SP) is an Electronic Health Records (EHR) and practice-management software used by therapists and other health and wellness professionals. Features include scheduling and designated client portal on the patient facing side. For clinicians, SP features documentation (EHR), telehealth, and options for insurance billing. The platform is used by 250,000 practitioners, serving over 20 million clients as estimated by SP. Prices were updated last in March of 2025 and did not increase in 2026. Monthly pricing points include three tiers ranging from $49 to $99. As itemized below, the monthly bill increases with add-ons including credit card fees and insurance claim fees.

Three plan options

Plan Price/month Promo (first 3 months) Built for
Starter $49 $24.50 Bare-bones solo practice
Essential $79 $39.50 Most solo practices
Plus $99 $49.50 Groups (required) and advanced features

Prices have been updated as of August 2026. Fees are billed monthly with no annual billing option. No free tier is available for users, but there is a free-trial option. Plans can be changed/updated at any time with prorated charges or credits.

Telehealth is included on all three plans. There are no limits to session length or session count for telehealth. Every plan carries the client portal with online booking. Each plan affords an ooptional professional website with directory listing. As an EHR, SP offers measurement-based care with notes, and online payments.

Starter plans miss out on workflow options. Appointment reminders exist only for telehealth sessions in the entry plan and there is no secure messaging. Other helpful tools that are not included in the Starter plan are options for calendar syncing or waitlist analytics. SP also drops Wiley Treatment Planners and template customization from the Starter plan through intake forms can be customized.

The Essential plan adds all of the above workflow options. Secure communication add-ons include full text and email. There are voice reminders included as well as 10 free insurance claims per month. Many clinicians consider the Essential plan as a minimum for private practice.

Plus features group appointments with up to 15 clients. This includes group telehealth. There are automatic insurance status checks with 35 free insurance claims each month. This is the only plan than offers users a premium support line.

Start the 30-day free trial to test a tier before the promo window decides your price.

Group practice pricing

SP offers group practices Plus with no additional plan options. This plans base rate is $99 a month for the first practitioner, then $74 per additional clinician up to 5 clinicians. The rate reduces slightly after 5 clinicians to $72 a month for up to 15 clinicians. Group billing rate for more than 16 clinicians is $69 per provider. Non-clinician team members (schedulers, billers, supervisors) are free with no cap. There is a fee for all-access practice managers at $39 a month each. Promo codes are good for group practices with rates at $49.50 for the first practitioner and $37 per additional for the first 3 months. At 6+ practitioners, the pricing page routes you to a sales call for custom pricing. A 3-clinician group therefore starts at $247 a month ($99 + $74 + $74) before fees, and the annual $20 AMA fee applies per clinician.

The fees on top of the subscription

  • Insurance claims. Starter pays $0.50 per electronic claim. Essential includes 10 a month, then $0.35 (falling to $0.29 above 1,000). Plus includes 35, then $0.25 (falling to $0.19 above 1,000). Insurance status checks are automatic on Plus, $0.15 each on Essential, $0.25 on Starter.
  • Card processing. 3.15% + $0.30 per successful transaction, up from the 2.95% third parties reported in 2025.
  • AMA CPT fee. $20 per clinician per year, charged to every account because CPT codes are accessible in the software, used or not.
  • AI add-ons. Care Aide (the full AI suite: Note Taker, Session Sidekick, a Treatment Planner beta, and Client Summaries) runs $59 a month for the owner and $49 per additional clinician after a free 30 days; Note Taker alone is $35 a month per clinician. Both carry half-price promos at the moment, and a June 16, 2026 policy change to Note Taker’s transcript retention is worth reading before turning it on.
  • ePrescribe. $49 a month per clinician plus a one-time, non-refundable $89 setup fee.
  • Group appointments on Essential. A $20-a-month per-clinician add-on; included on Plus, absent on Starter.

SimplePractice supports no eFax and has no public API, a workflow constraint to check before migrating. Its terms state it “does not offer refunds,” with cancellation stopping future charges only, and a closed account gets about a 60-day window to export client records before data is removed.

SimplePractice pricing in 2026 vs before

No 2026 price increase exists; the current numbers date from March 3, 2025, when the Starter plan jumped from $29 to $49 a month. This was a 69 percent increase, with roughly a month’s notice, while Essential and Plus held in price point. The company went private under Vista Equity Partners in the roughly $4 billion EngageSmart deal that closed in January 2024. Most commentators reference price increases to that buyout. Directory listings that predate the change still show $29 and $69 tiers, but SP’s own pricing page showed $49, $79, and $99 on August 29, 2026, and the numbers above match it.

Trustpilot holds SP at 3.0 of 5 across 1,608 reviews. These reviews spotlight pricing complaints which recur across 2026 reviews: “price increases, expensive add ons that are unwanted but they require you to purchase, sneaky add ons like their AMA annual fee,” as one August 2026 reviewer put it. A March 2026 reviewer allowed that “Simple Practice has made lots of improvements lately but gosh do they feel expensive,” and a June reviewer wrote that technical capabilities “cost a disproportionate amount.” Therapists on Reddit voiced the same complaints after the Starter hike.

Ways to pay less

  1. Take the 30-day free trial with no card required. This trial includes all features. Start the paid plan inside the trial’s first 7 days for the 50%-off of 3 months promotion.
  2. Cash-pay solo practices lose little on Starter. Insurance billers appear to save money on Essential through free claims and reminders.
  3. A referral program credits $200 after a referred colleague’s third payment. Each party earns this credit.
  4. If you are switching from another EHR; onboarding, data transfer help, and credentialing are free, with no migration charge listed.

How SimplePractice pricing compares

TherapyNotes Solo runs $69 a month with claims at $0.14 each (plus $0.14 per ERA), text reminders at $0.14 apiece, and card processing at 3.1% + $0.30. A solo therapist filing 40 claims a month lands around $89.50 at SP on Essential versus about $80.20 at TherapyNotes before reminders. From both fee schedules, we calculate in SP’s favor with free reminders, included telehealth (TherapyNotes charges $15 a month for its premium version), and customizable note and treatment plan templates with Wiley Treatment Planners. Our TherapyNotes vs SimplePractice comparison provides a full matchup.

For groups, TherapyNotes runs $79 for the first clinician plus $50 per additional clinician, with unlimited free non-clinical users, against SP’s $99 plus $74. PracticeQ, a newer alternative, lists $59.90 and $84.90 tiers with appointment caps per practitioner.

The all-in math for a typical insurance-billing solo practice on Essential: $79, plus around $10 in claims at $40 a month, plus $1.67 a month of AMA fee ($20 a year). That brings the total to roughly $91 before card processing and any AI add-ons. Read our SimplePractice review for another vantage.
