At a glance
  • Brand Zepbound costs $299 to $449 a month via self-pay at LillyDirect. The price depends on dose.
  • Compounded tirzepatide injections start with $99 first-month offers, and the lowest month-to-month price after that is $214. However, compounded drugs aren’t FDA approved.1
  • Prescriber fees may add to brand prices. Through Sesame Care, brand Zepbound totals $358 to $548 a month.

No generic tirzepatide is sold in the US, so online prices come from Eli Lilly’s brands or from compounding pharmacies. Lilly sells brand Zepbound self-pay through LillyDirect, its own online pharmacy. LillyDirect fills prescriptions but doesn’t write them. Sesame Care is a telehealth service whose providers can prescribe brand Zepbound and send prescriptions to LillyDirect. Your monthly total is the medication price for your dose plus any prescriber or membership fee.

A year of HealthRX paid monthly costs $2,453. It is $99 to sign up, then 11 months at $214. Prepaid, a year costs $2,123. Found bills every 28 days, so a year on its cash monthly plan is 13 charges of $359, or $4,667.

Zepbound and Mounjaro prices at LillyDirect

LillyDirect does not require or charge a membership fee, and its $299 starting price is the lowest self-pay brand price we found. Lilly counts a month as 28 days, which is four vials or one KwikPen.

Lilly says 2.5 mg is a starting dose and isn’t approved for maintenance, so the $299 price covers only the first weeks. Your prescriber sets the dose, and the pharmacy label shows it.

The 45-day refill rule

The $449 price for 7.5 mg to 15 mg requires each refill within 45 days of receiving the last one.2 A late refill costs $499 at 7.5 mg and $699 at 10 mg to 15 mg, so $50 to $250 more than the previous fill. A year of on-time refills at $449 is 13 fills, or $5,837.

Mounjaro self-pay price

Mounjaro costs $499 a month at LillyDirect at every dose. It’s FDA approved for type 2 diabetes, so prescribing it for weight loss is off-label.3 The same drug costs $50 to $200 less as Zepbound. Our Mounjaro coupon guide covers Mounjaro savings.

Adding a prescriber: Sesame Care and Hims

Sesame Care’s weight-loss membership costs $59 a month on an annual plan or $99 month to month, with medication billed separately. It covers video visits, unlimited messaging and Quest labs when needed. Labs aren’t included in nine states, including New York and New Jersey. On the annual plan, Zepbound totals $358 a month at 2.5 mg, $458 at 5 mg and $508 at higher doses. Some providers charge more than the base rate. Our Sesame Care review has the details.

Book a Sesame Care visit for a brand Zepbound prescription

Hims sells only brand tirzepatide, or Zepbound, from $299 a month. Its membership costs $39 the first month and $149 after that. Hims doesn’t show its prices for higher doses.

What is the cheapest tirzepatide online without insurance?

The cheapest tirzepatide online without insurance is the compounded injection from HealthRX.com, among the sellers we checked. It costs $214 a month after a $99 first month. HealthRX charges no membership or visit fee, and its 12-month prepaid plan works out to $177 a month.

The cheapest way to get brand Zepbound is a per-visit prescriber with no membership. Optum Perks Online Care charges $39 for a prescription request, and LillyDirect lists Counsel Health at $29 a visit. Add a $299 starting-dose vial or KwikPen and the first month costs $328 to $338.

Cheapest tirzepatide online with insurance or Medicare

Insurance only covers brand tirzepatide, not compounded formulations. In 2025, 19% of large employers offering health benefits covered GLP-1 drugs for weight loss.4 From 2025 to 2026, 12 million people lost coverage for Zepbound.5 Compounded medication is cash-pay at HealthRX.com, Henry Meds, Found and Mochi Health. Found’s insurance option bills the required video visit to your plan, typically about a $30 copay. With that option, compounded tirzepatide costs $149 for the first 28 days, then $289. A 12-month plan costs $179 per cycle.

Lilly savings card with commercial insurance

The Zepbound savings card lets commercially insured people pay as little as $25 for a 1- to 3-month fill of the single-dose pen. Savings cap at $100 a month and $1,300 a year, so $25 applies only if your copay is $125 or less. If your plan doesn’t cover Zepbound, the pen costs $499 or more with the card. Government plans such as Medicare, Medicaid and TRICARE are excluded, and the card expires December 31, 2026.2 Purchases with Lilly’s no-insurance KwikPen card can’t count toward an insurance deductible.2

Medicare GLP-1 Bridge

The Medicare GLP-1 Bridge charges eligible Part D members a $50 copay for Zepbound from July 1, 2026, to December 31, 2027.6 It covers only the KwikPen for weight management. Prior authorization is required. People with type 2 diabetes, moderate to severe sleep apnea or fatty liver disease aren’t eligible. Their Part D plan might cover a GLP-1.7

Sesame Care visits are self-pay, and its providers handle prior authorization paperwork for covered drugs.

How much does compounded tirzepatide cost?

Compounded tirzepatide costs anywhere from $177 to $389 a month at most sellers we checked, after first-month offers. The FDA doesn’t approve compounded drugs or review them for safety, effectiveness or quality.1 Since the tirzepatide shortage ended, 503A pharmacies may make it only in limited cases for individual patients.8

The cheapest compounded tirzepatide comes from sellers that aren’t Choosing Therapy partners. Sesame Care and Hims offer only brand drugs. Henry Meds sells compounded tirzepatide only as a dissolving tablet, and no oral tirzepatide is FDA approved.9 Its plan page calls it a 3-month supply but describes deliveries over 6 months, so the monthly cost isn’t clear.

Price by dose and what’s included

HealthRX and Found charge the same price at every dose. Mochi Health starts at $90 a weekly dose, or $360 for four weeks. Higher doses cost more, and membership is billed separately. HealthRX includes the clinician review, medication, overnight shipping and messaging, with a full refund if you aren’t approved. Found’s price covers care, medication and membership. Found has no plans in California. Ivim Health adds a $75 monthly program fee to medication starting at $117.

Compounded tirzepatide 3-month supply

A 3-month supply costs $602 at HealthRX with Subscribe & Save, about $201 a month. Found’s cash 3-month plan is $309 per 28-day cycle, or $927. Three months of brand Zepbound at $449 costs $1,347.

Price complaints in compounded tirzepatide reviews

Trustpilot reviews from late 2025 to October 2026 include complaints about price rises and extra costs. A September 2026 Mochi Health reviewer saw the price rise $29 at a higher dose. A January 2026 Found reviewer said a prescription cost wasn’t included in a plan they thought was all-inclusive.

A SkinnyRx reviewer reported a 6-month bundle auto-renewing at $1,776.60 before approval, later refunded. An Ivim Health reviewer in July 2026 named the monthly fee as a reason to switch. In April 2026, NPR reported on a patient who paid about $300 a month for compounded medicine after losing coverage. Lilly’s price for her Zepbound dose was $450.5

Red flags when tirzepatide looks too cheap

The lowest legal self-pay brand price is $299 for the starting dose. The FDA’s warning signs for online GLP-1 sellers include a company that:1

  • offers prices that seem too good to be true
  • claims its compounded drug is the same as an FDA-approved drug
  • skips the screening and prescription from a licensed doctor
  • has no licensed doctor for questions after delivery
  • uses labels with spelling errors, wrong pharmacy addresses or suspect pharmacy names

The FDA has warned sellers of tirzepatide labeled “for research purposes” or “not for human consumption.” The FDA urges consumers not to buy these products.1 It has also found compounded tirzepatide labeled with pharmacies that don’t exist. By May 31, 2026, it had more than 730 adverse event reports tied to compounded tirzepatide.1 In March 2026, it warned 30 telehealth companies over marketing that implied compounded GLP-1s match approved drugs.10

The FDA advises against using any injectable GLP-1 that arrives warm.1 Its BeSafeRx tool links to state pharmacy license lookups.11 NABP says nearly 95% of sites selling prescription-only drugs online operate illegally. It also warns against sites taking only cryptocurrency.12

How to get a tirzepatide prescription online

Tirzepatide needs a prescription from a licensed clinician, in person or by telehealth. Brand prescriptions go to LillyDirect or another pharmacy, and compounded ones to a state-licensed 503A pharmacy. Our guides to Zepbound, tirzepatide and getting Mounjaro for weight loss cover the steps.
