Although the platform stated in 2024 that its assessment was adapted for children (ages 6–12) and teens (13–17), information on pediatric services is limited, and we recommend other providers for non-adult care.1

An initial assessment with ADHD Online costs $189 and involves a comprehensive questionnaire reviewed by a licensed psychologist. No live session is included, but results are widely accepted by physicians.

Treatment options include medical management ($175–$199 per session, with or without medication), psychotherapy ($220 per session), and wellness coaching ($99 per session). ADHD Online stands out for its ADHD specialization, unlike broader mental health platforms such as Talkiatry and Brightside. ADHD Assessment & Diagnosis

One of the main offerings from ADHD Online is its unique virtual ADHD assessment and diagnostic system through Mentavi. For $189, patients can complete a detailed questionnaire screening for ADHD and co-occurring disorders, such as binge eating disorder, chronic loneliness, and problematic social media use. Payment plans are available.

ADHD Online says its ADHD assessment is built on credible, recognized tools like the ASRS-2 (a clinical ADHD assessment), PHQ-9 (depression screener), GAD-7 (anxiety assessment), Bergan’s Scale (measuring social media addiction), and additional screeners for conditions like binge eating disorder and chronic loneliness.

The clinical criteria for attention-deficit/hyperactivity disorder (ADHD) in the Diagnostic and Statistical Manual of Mental Health Disorders (DSM-5) is built into the questionnaire as well, so the psychologist that reviews your assessment can see how your current symptoms line up with established diagnostic criteria.1

ADHD Online bases its diagnostics on written responses, which can certainly provide valuable insights to the psychologist who reads them. However, some nuance is inevitably lost when compared to face-to-face evaluations. Having a provider in front of you to clarify and answer questions is ideal, and it’s something ADHD Online lacks.

Nonetheless, this method of ADHD assessment is probably preferable or even ideal for some. In-person treatment can be time-consuming, expensive, and hard to find. While ADHD Online may lack some nuance, it could be a game-changer for individuals with limited access to care.

Therapy, Wellness Coaching, & Psychiatry

If a patient is diagnosed with ADHD through the ADHD Online assessment, they will have the option to sign up for psychotherapy, wellness coaching, or medical treatment, which could include medication. The availability of these services varies by state, as do the providers who offer them; however, therapy with ADHD Online is now available in 40+ states.

Psychiatry & Medication Management

I decided to schedule an appointment with one of ADHD Online’s medication providers. Upon signing up, I noticed a limited selection of just four providers in my area, including two MDs and two nurse practitioners. Of those, only one had a fully detailed bio with information about their background and approach. Ultimately, my choice came down to scheduling.

I secured an appointment within a few days of receiving my ADHD diagnosis via the online assessment. The session took place over Zoom, which ADHD Online uses for all telehealth appointments. I joined from my iPhone using my home WiFi. Although I experienced a brief audio issue that required me to leave and rejoin the call, the rest of the session went smoothly.

My session was scheduled for 30 minutes but lasted about 15, during which my prescriber gathered basic information about my background, daily routine, and the symptoms I’d been experiencing. She also asked for vital signs and information like my height, weight, and blood pressure. It seemed like she primarily relied on the information from my initial assessment.

After reviewing my case, she recommended Adderall, a stimulant medication and federally controlled substance. She addressed my concerns about potential side effects and the risk of dependence, but I would have appreciated a broader conversation about other ways to manage my symptoms. Once I agreed to move forward, she sent the prescription to my local pharmacy.

Note that medication treatment with ADHD Online isn’t available in all states, and stimulant medications are even more restricted, so your location may influence the treatment options your provider can offer.

Therapy & Wellness Coaching

ADHD Online offers therapy with licensed therapists in 40+ states priced at $220 per session. Wellness coaching, which costs $99 per session, is available in all 50 states. In my region, therapy wasn’t an option, and while there were only two coaches to choose from, they offered a wide range of appointment times and dates. Coaching sessions are conducted via Zoom and providers are either licensed therapists or certified wellness coaches.

Mentavi’s Mental Wellness Snapshot is a brief, 20-minute quiz available for $29, which gives you access to the assessment for a full year. You can take the assessment as many times as you want within that time frame, giving you the ability to track any relevant symptoms over time. The assessment scans for anxiety, depression, PTSD, substance use disorders, ADHD, and more. The trackable score will also let you know if you may need more comprehensive treatment to address your symptoms.

Cost & Insurance

The Mentavi assessment with ADHD Online costs $189. In some states, medical treatment, which may or may not include medication prescription and management, is available after the assessment. This costs $199 for an initial consult, and $175 per follow-up. Wellness coaching (available nationwide) costs $99 per session, and psychotherapy (in 40+ states) costs $220 per session.

“I personally signed up for ADHD Online, paid the $189 fee, and completed the Mentavi Smart Assessment, an experience that gave me firsthand insight into how its online diagnostics work. I went through the entire process, from answering the 30-minute  questionnaire (which you can complete in one sitting or over a period of days) to receiving a detailed 500-word report from a psychologist within two days.

While I found the platform affordable and accessible, I noticed some gaps in care. For example, not all services are available nationwide, though you can receive a diagnosis and coaching in all 50 states. I also missed having a direct face-to-face interaction with a diagnosing clinician, though some patients might prefer it.

ADHD Online was founded in 2018 by Zachariah Booker and Dr. Randall Duthler as an asynchronous assessment platform that allowed people to be evaluated online without any real-time clinician interaction. Both founders, personally affected by ADHD, wanted to address the challenges they faced in finding timely and affordable diagnoses. In 2019, they also formed Mentavi Health, an affiliated company, and the originator of many additional comprehensive online mental health assessments.2

In 2021, the company experienced significant growth, quadrupling its corporate team and expanding its network of providers nationwide. This expansion allowed ADHD Online to serve patients with additional services across more US states.3

In 2023, a major corporate restructuring led to the formation of a new corporate entity with Mentavi Health becoming more like a parent company to ADHD Online. Keith Brophy, previously the Chief Operating Officer, was appointed CEO of both companies, succeeding Booker, who remained on the board of directors.4

In May 2025, Mentavi Health announced the results of a validation study that confirmed the reliability and accuracy of the Mentavi Smart Assessment for ADHD and other mental health conditions. Mentavi Chief Medical Officer, Barry K. Herman, MD, MMM, presented the findings at the 10th World Congress on ADHD. The study’s manuscript will be published later in 2025 after undergoing peer review.5

According to a Linkedin post from ADHD Online CEO, Keith Brophy, ADHD Online and Mentavi use AI in the background to “enhance support, streamline operations, and elevate patient experiences.” He clarifies that they do not use AI to diagnose or treat patients. He also says that Mentavi has its own AI Governance & Ethics Committee.7
