I used Ensora Mental Health for a week. I found Ensora’s recent improvements meaningful, particularly in areas like billing, payment processing, and the built-in telehealth suite. I think the revised pricing structure makes it more accessible for many practices. The customizable dynamic forms and client record system give you real control over your documentation, and the platform’s automation tools (available in Advanced and Premier tiers) help reduce admin load as your practice grows.
While Ensora may not have the largest app ecosystem or the strongest library of prebuilt templates, it strikes an excellent balance between affordability and flexibility. This is especially true for group practices, which can take advantage of unlimited client access and shared admin tools without paying for extra licenses. Do keep in mind the price escalates rather quickly with multiple providers.
Praise aside, Ensora’s not the only game in town. If you want a wider network of integrations and marketing/business tools, SimplePractice might be a better fit. Client Management
Ensora offers a straightforward but effective client management system that supports individual and group practices alike. From the central Clients tab, you can view a sortable, searchable list of active and inactive clients, with key details like name, status, and last appointment date visible at a glance. Clicking into a client profile opens a well-organized chart that includes tabs for notes, forms, documents, billing, and contact information, making it easy to find everything you need in one place.
The client face sheet highlights core identifying info, appointment history, and important documentation in a single, scrollable view. While it’s not radically different from what you’d find in other EHRs, it’s clean and easy to use, with quick links to upload files, send messages, or start a session.
Ensora also includes a secure client portal where clients can view appointments, documents, and invoices, and send messages. Advanced and Premier plans add online booking, which allows clients to self-schedule based on therapist availability, as well as secure messaging, helping streamline communication without relying on outside platforms.
Overall, Ensora’s client management tools don’t try to reinvent the wheel, but they’re well executed and bring over everything that worked about TheraNest’s client management system. Ensora Mental Health earned a full 5 out of 5 stars in this category.

Ensora’s intake system is flexible, customizable, and a clear evolution of the tools once offered by TheraNest and MyClientsPlus. Even on the more limited Essentials plan, practices have access to a set of prebuilt digital intake forms and notes. While the exact customization level at this tier isn’t spelled out, the essentials cover the basics for onboarding clients and capturing session information in a compliant and streamlined way. However, it definitely wasn’t the largest selection of prebuilt forms right out of the box.
Where Ensora starts to come into itself is with its Advanced and Premier pricing plans, which unlock full access to the platform’s dynamic form builder. This powerful drag-and-drop tool allows therapists to create fully customized intake packets, progress notes, and case documentation using conditional logic, reusable components, and smart templates. Unlike the old TheraNest model I used in the past, where some forms (like the intake/diagnosis form) were fixed, Ensora now lets users port any form into the dynamic builder for full control and consistency across documentation.
The unique Cases system has also carried over from TheraNest, offering a structured way to organize documentation by client goals or treatment areas. Each client can have multiple open or archived cases, which can help segment treatment notes, plans, and assessments for better clarity over time.
The overall intake experience feels modern, fluid, and well integrated into the broader workflow. It’s easy to build out form bundles, tie documentation to specific appointment types, and deploy everything through the secure client portal. I’d say Ensora now stands out as one of the more customizable intake systems in the EHR market, especially for teams that want consistency without giving up flexibility. It earned 4.5 out of 5 stars based on our scoring criteria.
Calendar & Scheduling
Ensora’s scheduling tools carry over many of the strengths of TheraNest while smoothing out some past frustrations from TheraNest’s early days. The interface is now clean and easy to navigate, with a crisp calendar view that supports multiple layouts and color-coded appointments by provider or session type. Drag-and-drop rescheduling works reliably, and appointment types can be customized to reflect telehealth, in-person, group, and other session formats.
Availability management works as you’d hope. Providers can create recurring or one-off availability blocks with fine-tuned control, and it’s easy to distinguish between different service locations. Ensora also handles time zone differences effectively: clients always see appointment slots in their local time, making it easier for practices with regional or national reach.
The client portal supports self-scheduling based on provider availability and syncs seamlessly with the calendar view. Email reminders are standard, and practices on the Advanced or Premier plans receive 30 or unlimited text and phone reminders (depending on tier), which can help reduce no-shows. Group practices also benefit from shared calendar features and a central view of provider availability across the organization.
While the system certainly doesn’t break a lot of new ground, it’s robust, flexible, and polished. It should meet the needs of most solo and group practices without requiring workarounds or third-party scheduling tools. For these reasons, Ensora Mental Health deserves a full 5 out of 5 stars in this category.
Treatment Planning & Notes
Ensora offers a solid, flexible system for clinical documentation, with strong customization options and clean integration across its note-taking and treatment planning tools. Progress notes can be built from scratch or adapted from included templates; currently limited to a standard note and a SOAP note by default. For many clinicians, these will serve as a functional baseline, but the real strength lies in the Dynamic Forms builder we explored earlier.
The Dynamic Forms tool allows you to create or modify note templates with drag-and-drop fields, conditional logic, and reusable components. It’s intuitive and powerful, especially once you’ve had a little time to explore it. Any note template, intake form, or assessment can be built within the same system, giving Ensora a coherent and modular documentation workflow.
Treatment plans remain slightly less customizable. They follow a set format with editable goals, objectives, and interventions, but you don’t get the same template flexibility that’s available for notes. That said, the plans are serviceable, and practices wanting a deeper library of structured templates can opt into the Wiley Practice Planner add-on ($25 monthly per therapist), which includes 1,000+ pre-written notes and planning materials.
Overall, Ensora strikes a very strong balance between flexibility and structure, with a clear emphasis on customization. It may require some initial setup to tailor the platform to your practice, but the tools are more than capable once configured, and light years ahead of what TheraNest once offered.

Sessions can be launched directly from the calendar or client profile, and clients join via the secure client portal, where they’re held in a virtual waiting room until admitted by the therapist.
The platform offers individual and group telehealth, with support for up to 20 participants in a session. Features include in-session chat, screen sharing, customizable backgrounds, and standard moderation tools like mute and disable video. Everything lives within the EHR environment, no need to juggle external links or separate tools.
Pricing remains competitive: telehealth is included on the Premier plan and available as a $12 monthly per therapist add-on for Essentials and Advanced users. Ensora’s telehealth system is reliable, convenient, and integrated tightly into the rest of the EHR; a major win for therapists who want one less platform to manage.

All the basics are covered: primary and secondary claims, batch submissions, rejection tracking, and real-time eligibility checks.
Practices on the Advanced plan receive 30 free claims and eligibility checks per month per practice, with additional usage billed at $0.23 per claim and $0.25 per eligibility check. Premier plan subscribers unlock unlimited access to these features, subject to a fair use policy.
Ensora also offers Revenue Cycle Management (RCM) services and Managed Billing Services for those who prefer to offload billing entirely. While pricing for RCM and managed billing isn’t publicly listed, it’s designed to serve practices of all sizes.

The platform includes an integrated billing dashboard for tracking charges, payments, superbills, and statements. Batch invoicing and bulk payments are supported, making it easy to process multiple transactions at once, especially helpful for group practices with high client volume.
Fees are pretty standard at 2.99% + $0.30 per transaction, and apply across all subscription tiers. There’s no option to use an outside payment processor, but Ensora’s built-in system is reliable and deeply embedded in the platform’s workflow.

These 15-minute sessions are easy to book and can be repeated as needed, making them a great resource for solo providers and growing teams alike.
The platform’s help center includes searchable documentation, how-to articles, and video tutorials that cover everything from calendar syncing to claim submission. Live webinars are also offered at no cost, and many are accessible even during the free trial period.
In our testing, we were contacted by a support representative shortly after account creation with a personalized welcome and an offer to help with setup. This quick response, paired with knowledgeable staff, makes for a smooth onboarding experience.
