8 Dimensions of Wellness Worksheet Overview
The 8 dimensions of wellness give you a way to look at every area of your life and see how each one is doing. Occupational therapist Peggy Swarbrick developed the model, and the Substance Abuse and Mental Health Services Administration (SAMHSA) shares it widely. Each dimension affects the others, so a struggle with money or sleep can spill into the rest.
This self-check explains all eight dimensions and asks you to rate each one for the past month. You will then look at the whole picture, see where you are strong, and choose one small step to start with.
In this worksheet, we’ll:
- Explain the eight dimensions of wellness, from emotional to spiritual
- Give you a table to rate each dimension and name one thing that would raise it
- Walk you through how a low score in one area can affect the others
- Provide space to pick one dimension and plan a small step
